Medical Treatment
I, as a new customer, hereby give my permission for Nancy Wallen and/or a member of the Fireplug Inn (FPI) staff to act as my agent in case of an emergency or in the case an any apparent medical or health issue regarding my dog(s).
In the event of any medical or emergency care issue which my arise while my dog(s) is (are) at FPI, I direct the staff of FPI to transport my dog(s) to one of the following, depending on FPI’s assessment of the situation: 1) our own personal veterinarian, 2) the nearest veterinarian clinic, or 3) an animal emergency care facility.
I agree to reimburse FPI for any charges it incurs in obtaining veterinary care, treatment, and/or medications or injections for my dog(s) while in FPI’s care.
Vaccinations
FPI requires current Rabies, Distemper-combination, and Bordetella preceding daycare, hotel, or grooming. I agree to provide updated vaccination records as needed. Those records can be faxed to 817-442-8309 or emailed to: nancy@firepluginn.com.
I NEED TO TALK TO MY VET ABOUT HOW OFTEN TO GIVE BORDETELLA.
- If my vet uses a 6-month vaccine, it must be updated every six months.
- If my vet uses an annual vaccine, the conversation with my vet needs to focus on what’s best for my dog, taking into consideration how often my dog comes to FPI and how susceptible my dog is to getting kennel cough. The vet may recommend giving Bordetella more often.
Release from Liability and Indemnity
I have toured FPI’s facility and am in agreement with its “open play” environment, outside exercise, and all of its policies and procedures. I agree to leave my dog(s) for daycare, boarding, or grooming at my own risk. I will not hold FPI, or anyone employed at FPI, responsible should an accident, illness, allergic reaction, loss, or death of my dog(s) occur while in FPI’s care, except if it arises out of FPI’s gross negligence or willful wrong.
I agree that if my dog(s) is not picked up within seven (7) days following the expected check-out date, and the staff has not been notified of a new check-out date, FPI has the right to consider the dog(s) abandoned. FPI will deliver the dog(s) to a dog rescue organization or an animal shelter.
Spay/Neuter
All dogs older than 6 months of age must be spayed/neutered before coming to FPI.
Fleas and Ticks
All dogs should be on flea/tick preventative. I understand that FPI makes every reasonable attempt to guard against flea/tick infestation.
However, I will not hold FPI responsible if my dog(s) come away from the facility with fleas or ticks. I also agree that if my dog(s) come into FPI with fleas or ticks, my dog(s) will be bathed and treated at my expense.
Miscellaneous Items
I agree that FPI is not responsible for lost or misplace personal items.
I understand FPI’s hours of business and will adhere to those hours of operation.
I agree to FPI’s rates for services rendered and agree to pay billed charges upon completion of services.
I understand that FPI allows dogs to socialize and play both indoors and outdoors and that FPI will make reasonable efforts to keep my dog(s) as clean as possible. However, I understand that my dog(s) may get dirty, and that I have the option of requesting and paying for bath services, if I choose.
I understand that FPI reserves the right not to continue serving dogs who are aggressive, unmanageable, too loud, or otherwise not suited to the FPI environment.
I understand that FPI will not release my dog(s) to any person other than those in my family. If someone else is to pick up, I will give FPI that person’s name and phone number.
